All grades:Up to date on immunizations  or a Medical or Religious Exemption (check with your student's medical provider or Hamilton County Public Health if unsure) 
Kindergarten: Dental Screening, Lead Screening,Physical, Vision Screening  prior to the start of the school year
3rd Grade: Vision Screening  prior to the start of the school year  
7th Grade: Tdap and Meningococcal Vaccines (prior to starting 7th grade, can be given anytime after age 11)
          Hamilton County Public Health offers a school based vaccine clinic for 6th graders who are eligible at the end of each school year, to receive their Tdap and Meningococcal Vaccines.  Information will be sent home with students prior to this school based clinic. 
9th Grade:  Dental Screening  prior to the start of the school year
12th Grade; Meningococcal Vaccine after age 16 (prior to starting 12th grade, can be given anytime after age 16)
        Hamilton County Public Health offers a school based vaccine clinic for 11th graders who are eligible at the end of each school year to receive their Meningococcal Vaccine.  Information will be sent home with students prior to this school based clinic. 


Athletic Preparticipation Physical Form (upload a picture of page 4 to BOUND App/website when complete): Required for each student (grades 7-12) participating in any school athletics, updated yearly.  First time Bound registration instructions.
Heads Up Concussion Form (complete on BOUND App/website): Required for each student (grades 7-12) participating in any school athletics, updated yearly.
Hearing screenings: The AEA conducts screenings of students with a history of known hearing loss, new students, and those with suspected hearing concerns as needed.
Hygeine and Puberty: Boys and Girls in 4th and 5th grades participate in separate class discussions about hygiene and normal body changes during puberty. Information will be sent home prior to this class discussion.
